What Arizona requires on your label

The label maker below handles all of this for you. Here is the checklist it follows, straight from Arizona's rules.

   Name of the food preparer

   The registered preparer's name (the statute requires the preparer's name; adding a product name is best practice).

   ADHS registration number

   No home address is required on the label; the registration number identifies you.

   All ingredients

   List everything in the product.

   Production date

   The date the food was prepared.

   ADHS reporting and verification website

   The department-provided website address for reporting foodborne illness and verifying registration status must appear on the label.

   Required disclaimer

   Exact wording below (updated by HB 2042, rules effective February 4, 2025).

• HB 2042 changed the disclaimer wording ("may come in contact with common food allergens and pet allergens"); older labels using the pre-2025 wording need reprinting.
• Text must be in a clear and legible printed or handwritten font; no minimum point size is specified.
• Net weight is not required by A.R.S. 36-932. Perishable (TCS) foods are now allowed with additional handling and registration conditions.
