    Required only if the product is made from two or more ingredients.

    Major food allergens
    Milk, eggs, fish, shellfish, tree nuts, peanuts, wheat, soybeans, and sesame.

    Name and contact information of the operator
    A street address is not required; the official sample label uses a business name, website, and email.
